Scope Addition - Service Site: North Chicago Health Center - Burke

As a federally qualified health center (FQHC), the Lake County Health Department and Community Health Center (LCHD/CHC) is required to maintain a scope of project with the Health Resources and Services Administration (HRSA). The scope defines the approved service sites, services provided, service areas and target populations that are served with section 330 grant funds. LCHD/CHC is responsible for updating this scope of project as organizational sites and services change.
The North Chicago Health Center (NCHC) had a 4-inch water line burst in the attic and subsequent flood in the late evening hours of January 22, 2024 and into the early morning of January 23, 2024. The health center was immediately closed. All NCHC staff were relocated to the Belvidere Medical Building. NCHC was removed from the LCHD/CHC scope.
Remediation and reconstruction work is in the final stages, thus the NCHC needs to be added back into the LCHD/CHC scope. The scope addition needs to be approved by the Governing Council. If Governing Council approval is obtained, LCHD/CHC staff will submit a formal service site scope addition request to HRSA.

To approve the service site scope addition, adding the NCHC service site to the LCHD/CHC scope as presented.
