Staff Summary

                     Pulaski Drive Bridge over US Rte. 41 Deck Repair and Resurfacing: Change Order No. 2

                     Change orders need to be approved by the standing committees for cumulative net increases over 10%.

                     Pulaski Drive Bridge over US Rte. 41 deck repair and resurfacing project is under an original awarded contract at bid unit prices totaling $306,779.42.

                     The reason for previous change order No. 1 was to install temporary pavement so that traffic has the proper tapers into the work zone.  Traffic will be placed on the outer two lanes while the inner two lanes are rehabilitated. Although the bridge is 4 lanes wide, the approach pavement is only 2 lanes wide.  Following construction, this pavement can remain in place to be used as paved shoulders or a temporary pavement for any future maintenance.

                     This change order No. 2 is for the balance of the work necessary to construct the temporary pavement.  It also includes increases to the partial concrete deck slab repair quantity to address additional deteriorated area that could not be anticipated until the old asphalt overlay was removed.

                     The original appropriation of $385,000 of County Bridge funds is exceeded.  An additional appropriation of $25,000 of County Bridge funds is necessary.  Summary:  Original awarded contract at bid unit prices: 306,779.42; Change Order No. 1 - $30,620.25; Change Order No. 2 - $62,562.67; sum of all changes - $93,182.92.
