· Proposals were called for by publication for Pre-Employment Physicals for Lake County.
· Requests for proposals were extended to six (6) vendors, sealed proposals were received from three (3) vendors, and the selection review Committee held interviews with one (1) firm.
· Based on the criteria set forth in the Request for Proposals; the Evaluation Committee selected Advocate Occupational Health as the most favorable proposal for Lake County.
· This contract will cost the County an estimated annual amount of $95,245 per year for a two (2) year period with three (3) one year renewals. Charging the costs to 206-1440010-71420.

WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, proposals were called for by publication for Pre-Employment Physicals for Lake County; and
WHEREAS, subsequent to said publication, requests for proposals were extended to six (6) vendors; and
WHEREAS, sealed proposals were received from three (3) vendors and opened in the County Building, Waukegan, Illinois, on June 30, 2010 at 2:00 p.m.; and
WHEREAS, an ad hoc evaluation committee held interviews with one (1) firm; and
WHEREAS, based on the criteria set forth in the Request for Proposals, the Evaluation Committee selected Advocate Occupational Health of Park Ridge, IL as the most favorable proposal for Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the Purchasing Agent is authorized to negotiate and enter into a contract with Advocate Occupational Health, Park Ridge, Illinois, in the estimated annual amount of $95,245 for a two (2) year period with three (1) year renewals for Pre-Employment P...
