Committee action to modify Contract #27002 for Employee Assistance Program and Disease Management Program services.
Staff Summary
· In April, 2007, the Lake County Board approved a contract with APS Healthcare, Silver Spring, Maryland in the estimated amount of $299,852. The contract bundled for Employee Assistance services (EAP) and Disease Management Program services (DM) for Lake County employees.
· Under the original contract Lake County paid $8.06 per employee per month for an integrated EAP and DM program.
· There is a need to issue Contract Modification #1 to un-bundle the contract services; to discontinue the DM program services, and change the price for EAP services from $1.71 per employee per month to $2.50 per employee per month.
· The circumstances that necessitate this Contract Modification were not reasonably foreseeable at the time the contract was signed. The changes are germane to the original contract as signed, are in the best interests of the County and authorized by law.
· The new un-bundled price for EAP services are considered reasonable and acceptable.
· This will reduce the estimated contract amount by $162,000.
· In accordance with Article 8, section 101 (1) of the Lake County Purchasing Ordinance, it is hereby requested that the Purchasing Agent be authorized to enter into Contract Modification #1.
