Lake County Health Department and Community Health Center (LCHD/CHC) received grant funding through the AIDS Foundation of Chicago for the Ryan White A Medical Benefits Coordination program in the amount of $57,245. The period of performance is April 16, 2024, to February 28, 2025. The purpose of this funding is to provide medical benefits coordination services. This will fund a new Case Manager position, supplies, contractual services, a computer, and offset an existing staff salary and fringe benefits.
