Lake County Health Department and Community Health Center (LCHD/CHC) received an increase in grant funding from the Illinois Department of Human Services (IDHS) for the Child and Family Connections (CFC) program in the amount of $276,101. The performance period is from July 1, 2024, to June 30, 2025. The purpose of this funding is to help identify and coordinate early intervention services for children. This will continue to fund two Service Coordinator positions and offset existing staff salary and fringe benefits.

To approve the Illinois Department of Human Services, Child and Family Connections grant funding increase and emergency appropriate $276,101 for FY24.
