The Lake County Health Department and Community Health Center presents the Lake County Health Department Strategic Plan for Council approval. Health department leadership and staff have collaborated to produce an open, adaptive strategic plan based on the three strategic themes: effective primary prevention, highest quality, and robust community-driven health neighborhood. The Board of Health approved the 2020 Strategic Plan on August 26, 2020. Through a 25-year lens, we are presenting four focus areas with ten 3 to 5-year objectives. Each objective has a measure of success by which the Board of Health and our community will know when we have achieved our objective.

To approve the Lake County Health Department and Community Health Center 2020 Strategic Plan as presented.
