Presented is a request for additional privileges for the three providers noted. The Medical Director has verified the additional privileges requested are appropriate for each provider’s training and experience.
The Medical Director has studied the application, raised questions where necessary, and based upon satisfactory responses from the applicants, recommends approval of the request for additional privileges.
