Health Resources and Services Administration (HRSA) Operational Site Visit (OSV) Findings and Responses - Peer

Lake County Health Department and Community Health Center (LCHD/CHC) underwent a scheduled virtual site visit and review from HRSA August 3-5, 2021. The review was to evaluate how well LCHD/CHC is meeting the required standards for Federally Qualified Health Centers (FQHC) related to Administration, Clinical Operations and Finance.
The review was very successful with only six findings within the Health Center requiring limited modifications, many of which were addressed prior to the conclusion of HRSA’s visit. The changes outlined in the attached presentation have been submitted to HRSA and final approval of corrective actions is pending.
