Lake County Health Department and Community Health Center (LCHD/CHC) received an increase in grant funding through the AIDS Foundation of Chicago, Ryan White B Case Management program in the amount of $6,936. The period of performance is April 1, 2024, to March 31, 2025. The amount requested to be appropriated in LCFY24 is $4,624. The difference has been accounted for in the LCFY25 budget. The purpose of this funding is to provide case management services. This will fund supplies and offset existing staff salary and fringe benefits.

To approve the AIDS Foundation of Chicago, Ryan White B Case Management grant funding increase of $6,936 and emergency appropriate $4,624 for LCFY24.
