Staff Summary
· Proposals were called for by publication for voluntary Vision Services for Lake County employees.
· Requests for proposals were extended to eight (8) vendors, sealed proposals were received from five (5) vendors, and an ad hoc Committee held interviews with two (2) finalists.
· Based on the criteria set forth in the Request for Proposals and the interview, the ad hoc Committee selected Superior Vision as the most favorable proposal for Lake County.
· This contract is 100% employee paid.
Body
WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, proposals were called for by publication for voluntary Vision Services for Lake County; and
WHEREAS, voluntary Vision Services are services offered to employees to supplement the vision benefit/discount offered through the County’s health insurance plans; and
WHEREAS, subsequent to said publication, requests for proposals were extended to eight (8) vendors; and
WHEREAS, sealed proposals were received from five (5) vendors and opened in the County Building, Waukegan, Illinois, on June 23, 2010 at 2:00 p.m.; and
WHEREAS, an ad hoc evaluation committee with representation from the Sheriff’s Office, Human Resources, Health Department and Forest Preserve reviewed all proposals and held interviews with two (2) finalists; and
WHEREAS, based on the criteria set forth in the Request for Proposals, the ad hoc Evaluation Committee selected Superior Vision of Milford, MI as the most favorable proposal for Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the Purchasing Agent is authorized to negotiate and enter into a contract with Superior Vision of Milford, MI, for a two (2) year period plus renewals for voluntary Vision Services for Lake County.
