Staff Summary
· The Human Resources Department desires to enter into a contract for an Employee Assistance Program.
· Request for Proposals were sent to 214 vendors and nine were received.
· A selection committee comprised of Human Resources, the 19th Judicial Circuit Courts, Forest Preserve and Purchasing recommended ComPsych Corporation of Chicago, Illinois as the firm that best meets the needs of the County based on the RFP evaluation criteria.
· The contract authorizes the Purchasing Agent to enter into a two-year agreement with three additional one-year renewal options that will cost the County an estimated amount of $60,912.

WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing
Ordinance, Request for Proposals were called for by publication for an Employee Assistance Program; and
WHEREAS, subsequent to said publication, Request for Proposals were extended to 214 potential vendors; and
WHEREAS, sealed proposals were received from nine vendors and opened in the County Building, Waukegan, IL on April 10, 2012 at 2:00 p.m. and
WHEREAS, based on the criteria set forth in the Request for Proposal for an Employee Assistance Program it has been determined that the proposal submitted by ComPsych Corporation is the most favorable for Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County,
Illinois, that the Purchasing Agent is hereby authorized to enter into a contract with ComPsych Corporation in the estimated amount of $60,912 for an Employee Assistance Program charging the costs thereof to account #510-1430010-74040.
