As defined in the Governing Council bylaws, one of the responsibilities assigned to the Council is the approval of adding/changing/removing service delivery sites for the Lake County Health Department and Community Health Center (LCHD/CHC) Federally Qualified Health Center. The attached slides detail: zip codes of Lake County residents living under the federal poverty level, zip codes of LCHD/CHC FQHC patients, wait list, open general medicine provider positions, North Shore Health Center (NSHC) patient zip codes for medical and dental services, NSHC building expenses, and alternative medical and dental sites in the NSHC area.
Suggested Motion
To remove North Shore Health Center from LCHD/CHC Scope.
