Licensed Independent Practitioner (LIP) Privileging and Credentialing - Steres

Presented are two (2) LIP for privileging and credentialing. The proper verifications have been carried out and there were no identified issues. The Medical Director has verified the privileges requested by the provider are appropriate for the provider’s training and experience. The Medical Director, the Board of Health Executive Committee, and the Governing Council Personnel Committee has studied the application, raised questions where necessary, and based upon satisfactory responses, recommends approval of the request for privileging and credentialing.
