Emergency Appropriation of $259,394 for Illinois Department of Healthcare and Family Services (HFS), Care Coordination and Support Organization (CCSO) - Esser

Lake County Health Department and Community Health Center (LCHD/CHC) received notification from the Illinois Department of Healthcare and Family Services that increased reimbursement rates became effective August 2024 to help support the sustainability of the Pathways to Success Care Coordination and Support Organization program (CCSO). Along with this rate increase, the number of referrals has been growing. Three new positions are needed to meet the demand of the referrals and adhere to HFS requirements. The funding amount to be appropriated is $259,394 for LCFY26. The CCSO is a program for Medicaid enrolled youth under the age of 21 who have complex, high-risk behavioral health needs and require evidenced-informed intensive care coordination along with home and community-based services to supplement existing treatment interventions. This will fund three new positions.
