Presented are seven (7) LIP for initial appointment and reappointment. The Medical Director has verified the privileges requested by the provider are appropriate for the provider’s training and experience. The Medical Director and the Board of Health Credentialing Committee have studied the applications, raised questions where necessary, and recommend approval.
