Staff Summary
· In October 2011, the Lake County Board authorized a contract with CallOne Communications for network communication services for ETSB in the estimated amount of $61,800.
· The amendment to the services is required to install the VPLS Fiber connection at the ETSB office in Volo, IL due to the closing of dispatch centers in Libertyville and Antioch.
· Amendment #1 increases the dollar amount by $35,200 this changes the previous estimated amount from $61,800 to a total of $97,000.
· In accordance with Article 8, Section 101(1) of the Lake County Purchasing Ordinance it is hereby requested that the Purchasing Agent be authorized the execution of Amendment #1 to Lake County Agreement #11132.

RE: Contract Modification Agreement #11132
1. In October 2011, the Lake County Board authorized a contract for Communication Network Services with CallOne Communications, Chicago, IL in the estimated amount of $61,800.
2. This amendment increases the dollar amount of the Agreement #11132 by $35,200, increasing the total from $61,800 to $97,000.
3. The VPSL fiber connection services were not included under the original proposal and all other terms and conditions of the Agreement remain the same.
4. In accordance with Article 8, Section 101(1) of the Lake County Purchasing Ordinance, it is hereby requested that the Purchasing Agent be authorized to execute Amendment #1 to Lake County Agreement #11132.
5. This change order will cost the County an amount not-to-exceed $97,000.
