Presented are two (2) providers for credentialing and privileging. The Medical Director has verified the privileges requested are appropriate for the provider’s training and experience. The Medical Director, the Board of Health Credentialing Committee, and the Governing Council Personnel Committee has studied the applications, raised questions where necessary, and based upon satisfactory response, recommends approval of the request for privileges.
