Staff Summary
• On April 12, 2016, the County Board ratified a professional services contract and Change Order Number One in the amount of $58,412 with Rezek Henry Meseinheimer Engineers, Inc., Mundelein, Illinois, in accordance with the Local Government Professional Services Selection Act.
• The scope of work under this contract included design, bidding, and construction services for the reservoir improvements and a second set of bidding documents for the abandonment of well number seven for the Bridlewood Reservoir Improvements Project.
• Change Order Number One, for additional services, was required as a result of direction received during a variance hearing with the Lake County Zoning Board of Appeals for site revisions to Central Lake County Joint Action Water Agency (CLCJAWA) proposed improvements to the Lake Michigan Water Project, and the abandonment of well number six.
• Change Order Number Two in the amount of $4,206, increases the previous contract amount from $58,412 to $62,618, for work items that were not included under the original agreement but are germane to the contract.
• In accordance with Article 8, Section 101 (1) of the Lake County Purchasing Ordinance, it is hereby requested that the Purchasing Agent be authorized to enter into contract modification for Change Order Number Two.

1. On April 12, 2016, the County Board ratified a professional services contract and change order number one in the amount of $58,412 with Rezek Henry Meseinheimer Engineers, Inc., Mundelein, Illinois, in accordance with the Local Government Professional Services Selection Act.
2. The scope of work under this contract included design, bidding, and construction services for the reservoir improvements and a second set of bidding documents for the abandonment of well number seven for the Bridlewood Reservoir Improvements Project.
3. Change order #1 for additional services was required as a result of direction received during a variance hearing with the Lake County Zoning Board of Appeals for site revisions to CLCJAWA proposed improvements for the Lake Michigan Water Project, and the abandonment of well number six.
4. Change Order #2 is in the amount of $4,206 increasing the previous contract amount from $58,412 to $62,618 for work items that were not included under the original agreement but are germane to the contract.
5. This modification will cost the County an amount not-to-exceed $4,206 charged to account # 610-4843506-71170-000-000-000-48310.
