Staff Summary
• The Health Department has received an increased number of referrals for the Care Coordination and Support Organization program for the period of December 1, 2025, through November 30, 2026.
• The $259,394 Illinois Department of Healthcare and Family Services program funding will be used to fund three new positions to meet the demand of the referrals and adhere to the Illinois Department of Healthcare and Family Services requirements.
• This funding is to support the program for Medicaid enrolled youth under the age of 21 who have complex, high-risk behavioral health needs and require evidenced-informed intensive care coordination along with home and community-based services to supplement existing treatment interventions.
• If funding for the positions and services end, and new funding is not secured, the positions and services will be eliminated.

RESOLUTION
WHEREAS, the Lake County Health Department and Community Health Center has received an increase delivered by our Clinical Operations Unit; and
WHEREAS, these dollars will provide an increase in services to Lake County with no impact on local taxes; and
WHEREAS, this increase will be used for three new positions that will be eliminated if funding is no longer available; and
WHEREAS, the additional funding amount totaling $259,394 for the period of December 1, 2025 through November 30, 2026, has not been previously appropriated, having been approved after adoption of the Fiscal Year 2026 budget; and
WHEREAS, the proposed appropriation will be fully offset by the above referenced items thereby not requiring the expenditure of local tax revenue.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that an emergency appropriation in the amount of $259,394 is hereby authorized in the Health Department Fiscal Year 2026 budget as identified on the listing of expense and revenue budget modifications attached hereto and made part hereof; and
BE IT FURTHER RESOLVED, that the revenue budgets as identified on the attached listing be modified as provided by increased funding from the following:
Illinois Department of Healthcare and Family Services - Care Coordination and Support Organization, $259,394.
