Staff Summary
·      In July 2008, Lake County entered into a contract with Campanella and Sons Inc. of Wadsworth, Illinois to perform Site Work Construction Services for the Central Permit Facility and Consolidated Environmental Laboratory in Libertyville in the amount of $2,576,828.60.
·      In October 2008 Change Order #1 was deducted from the contract.  This change order included additional costs of $8,989 for miscellaneous site revisions and a credit amount of $24,723.60 for omission of site signage and some campus walkways, reducing the contract amount by $15,734.60.
·      In October 2008 Change Order #2 was added to the contract to incorporate access road and partial parking area undercuts using unit prices in an estimated amount not-to-exceed $231,787. The estimated quantities of porous granular embankment, geotextile fabric and undercut excavation were determined based on the field testing completed at the time of the change order.
·      There is a need for Change Order #3 relative to additional required cut and fill areas identified since the execution of Change Order #2.
·      Campanella and Sons, Inc is the appropriate party to assume these contractual requirements. These changes are germane to the original contract, in the best interests of the County and authorized by law.
·      In accordance with Article 8, section 101 (1) of the Lake County Purchasing Ordinance, it is hereby requested that the Purchasing Agent be authorized to enter into Contract Modification Change Order #3 in the amount not-to-exceed $173,622 with Campanella and Sons, Inc, Wadsworth, Illinois charging the costs thereof to account 101-1103065-82010-000-000-000-00000.

1.      Effective July 2008, Lake County entered into a contract with Campanella and Sons Inc. of Wadsworth, Illinois to perform Site Work Construction Services for the Central Permit Facility and Consolidated Environmental Laboratory in Libertyville. The original contract was awarded in the amount of $2,576,828.60.
  1. Change Order #1 was deducted from the contract in October 2008. This change order included additional costs of $8,989 for miscellaneous site revisions and a credit amount of $24,723.60 for omission of site signage and some campus walkways. This change order reduced the contract amount by $15,734.60 resulting in a revised contract amount of $2,561,094.
  2. Change Order #2 was added to the contract in October 2008 to incorporate access road and partial parking area undercuts using unit prices in an estimated amount not-to-exceed $231,787.00. The estimated quantities of porous granular embankment, geotextile fabric and undercut excavation were determined based on the field testing completed at the time of the change order. This change order amount of $231,787.00 resulted in a revised contract amount of $2,792,881.00.
  3. There is a need for Change Order #3 relative to additional required cut and fill areas identified since the execution of Change Order #2.
 
A. The balance of the parking lot has been cut to subgrade and tested. This testing requires that additional cut and fill be performed due to existing soil conditions. The estimated quantities and unit prices are as follows:
·      Porous Granular Embankment Fill - 6,500 tons at $20.35/ton = $132,275.00
·      Undercut excavation - 3,250 CY at $6.30/CY = $20,475.00
·      Geotextile Fabric - 4,200 SY at $2.75/SY = $11,550.00
B. In addition to cut and fill requirements, it has become necessary to augment site soil erosion control measures based on the additional stockpile material. This will cost a lump sum of $9,322.00.
  1. Campanella and Sons, Inc is the appropriate party to assume these contractual requirements. These changes are germane to the original contract, in the best interests of the County and authorized by law.
  2. In accordance with Article 8, section 101 (1) of the Lake County Purchasing Ordinance, it is hereby requested that the Purchasing Agent be authorized to enter into Contract Modification Change Order #3 in the amount not-to-exceed $173,622 charging the costs thereof to account 101-1103065-82010-000-000-000-00000 with Campanella and Sons, Inc, Wadsworth, Illinois.  There is sufficient budget in that line item.
