• Lake County desires to enter into a contract with, Correct Care Solutions LLC, of Nashville, TN, for Health Cares Services for the Adult Correctional Facility and the Juvenile Detention Center.
• Request for Proposals were sent to nine (9) vendors and four (4) proposals were received.
• An evaluation committee selected Care Solutions LLC, of Nashville, TN, to be in the best interest of Lake County.
• This contract will cost the County an estimated amount of $1,967,650 charging the costs to account # 101-3007010-71330-000-000-000-00000; and for the Health Care Services for the Juvenile Detention Center in the estimated amount of $196,000 per year, charging $190,120 to account #216-3240010-71330-000-000-000-00000 and $5,880 to account #216-3240030-71330-000-000-000-00000, for a two year contract plus renewal options, in accordance with their proposal.

WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, proposals were called for by publication for Health Care Services for Lake County Adult Correctional Division and Juvenile Detention Center; and
WHEREAS, subsequent to said publication, request for proposals were extended to nine (9) vendors; and
WHEREAS, sealed proposals were received from four (4) vendors and opened in the County Building, Waukegan, Illinois, on Tuesday, July 22, 2008, at 2:00 p.m.; and
WHEREAS, based on the criteria set forth in the Request for Proposals, an Evaluation Committee selected the Care Solutions LLC, of Nashville, TN, to be in the best interest of Lake County.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the Purchasing Agent is hereby authorized to enter into a contract with Care Solutions LLC, of Nashville, TN, in the estimated amount of $1,967,650, for Health Care Services for Lake County Adult Correctional Division, charging the costs to account #101-3007010-71330-000-000-000-00000; and for the Health Care Services for the Juvenile Detention Center in the estimated amount of $196,000 per year, charging $190,120 to account #216-3240010-71330-000-000-000-00000 and $5,880 to account #216-3240030-71330-000-000-000-00000, for a two year contract plus renewal options.
