• Lake County holds a tract of land as Trustee (COLT Property), PIN 15-06-203-019, that exists adjacent to the parcel of land in Mundelein on which the Winchester House replacement facility is to be built.
• In order for the Winchester House replacement facility to be built, the Village of Mundelein requires that two separate water line connections be made to the facility, creating a loop.
• The property that Lake County holds as Trustee sits between the Village of Mundelein water main and the property on which the Winchester House replacement facility is to be built.
• Approval of the recommended utility line easement agreement will allow for a water line to be installed connecting the Village of Mundelein water system to the replacement facility for Winchester House.

R E S O L U T I O N
WHEREAS, Lake County holds a tract of land as Trustee (COLT Property) under 35 ILCS 200/21-90, which is described in the attached “Utility Line Easement Agreement,” located at an address commonly known as 25635 U.S. Highway 45, Mundelein, Illinois, 60060; and
WHEREAS, this tract of land has a PIN of 15-06-203-019; and
WHEREAS, this tract of land is immediately adjacent to the parcel on which Transitional Care of Lake County (TCLC), also known as Transitional Care of Mundelein, intends to build the replacement facility for Winchester House; and
WHEREAS, in order for the replacement facility for Winchester House to be built, it requires looped water line connections from two separate Village of Mundelein water mains; and
WHEREAS, in order for TCLC to gain access and connect to one of two available water mains, from the Village of Mundelein, a utility easement agreement is required so that a water line can be installed through parcel 15-06-203-019; and
WHEREAS, the County continues to support the replacement of the current Winchester House operation with a new skilled nursing facility in Mundelein, Illinois by acknowledging that approval of this easement agreement will assist in providing needed water service to that facility.
NOW, THEREFORE, BE IT RESOLVED, by the County Board of Lake County, Illinois that the County Board directs the County Administrator to sign a “Utility Line Easement Agreement,” in a form substantially similar to the document attached, between the County of Lake, Illinois and IH Mundelein Owner, LLC.
