Resolution recognizing the service and contributions of Charles Fornero, member of the Lake County Health Department and Community Health Center’s Governing Council on his retirement.

RESOLUTION
WHEREAS, Charles Fornero has served the Lake County Health Department and Community Health Center’s Governing Council since August 2017; and
WHEREAS, during his tenure he played a key role in the many successes of the Lake County Health Department and Community Health Center’s Federally Qualified Health Centers, and
WHEREAS, he served as the Governing Council Chairman from 2023-2025, Budget Committee Chair from 2021-2023, Vice Chairman 2019-2020, and
WHEREAS, his active role on the Governing Council provided guidance and support to the Lake County Health Department Federally Qualified Health Centers Leaders during the COVID pandemic, and
WHEREAS, he participated in and led the Governing Council in three successful Health Resources and Services Administration (HRSA) Operational Site Visits and two successful Joint Commission Surveys, and
WHEREAS, during his tenure as Governing Council Chair, he oversaw:
• The transformation of care model from a fee for service to value based care approach
• The improvement in patient care and quality outcomes with the Federally Qualified Health Centers being awarded as a Silver Health Center Quality Leader by HRSA in addition to receiving four other National Quality Leaders awards, and
WHEREAS, his focus, clarity of thought, dedication, drive and overall commitment to the residents of Lake County and specifically the patients of the Federally Qualified Health Centers as a member of the Governing Council and many more selfless, servant leader initiatives.
NOW, THEREFORE, BE IT RESOLVED, by the President and members of the Lake County Board of Health, that we do hereby recognize Governing Council Member Charles Fornero’s extensive contributions to the residents of Lake County through his dedication to the betterment of the Lake County Health Department’s Federally Qualified Health Centers, we express our heartfelt gratitude for his years of service and commitment to making the lives of others better, with a true commitment to the mission of the Lake County Health Department to “Promote the Health and Well-Being of all who Live, Work and Play in Lake County”; and
BE IT FURTHER RESOLVED that this Lake County Board of Health extends its best wishes to Governing Council Member Charles Fornero on his retirement and wishes him all the best in the years to come.
