Staff Summary
• Proposals were called for by publication for Section 125 Flexible Spending Program Services, Supplemental Insurance Services and third party administrative services for COBRA and Retiree billing services for Lake County employees.
• The Request for Proposal was divided into four (4) sections: Section I, Section 125 Flexible Spending Program Services, Section II, Accident and Cancer Supplemental Insurance Services, Section III, Retiree Billing Administration, and Section IV, COBRA Administration.
• Requests for proposals were extended to nine (9) vendors, sealed proposals were received from six (6) vendors, and an ad hoc Committee held interviews with three (3) finalists.
• Based on the criteria set forth in the Request for Proposals; the ad hoc Committee selected AFLAC, Columbus, GA for Section II, employee-paid Supplemental Insurance Plans and Healthsmart, Lisle, IL for Section I, III, and IV for the Section 125 Flexible Spending Program Services and Third Party Administrative Services for COBRA and Retiree Billing.
• The cost for COBRA and Retiree Billing services will be passed on to the participants.
• The Contract for Section 125 Flexible Spending Program Services will cost the County an estimated amount of $30,000, charging the costs thereof to account 730-1450010-74090.

WHEREAS, in accordance with Section 5-102 of the Lake County Purchasing Ordinance, proposals were called for by publication for Section 125 Flexible Spending Program Services, Supplemental Insurances, COBRA Administration & Retiree Billing Administration for Lake County; and
WHEREAS, the Request for Proposal was divided into four (4) sections:
Section I: Section 125 Flexible Spending Program Services
Section II: Accident & Cancer Supplemental Insurance (employee-paid)
Section III: Retiree Billing Administration
Section IV: COBRA Administration
WHEREAS, subsequent to said publication, requests for proposals were extended to nine (9) vendors; and
WHEREAS, sealed proposals were received from six (6) vendors and opened in the County Building, Waukegan, Illinois, on July 22, 2010 at 2:00 p.m.; and
WHEREAS, an ad hoc evaluation committee held interviews with three (3) finalists; and
WHEREAS, based on the criteria set forth in the Request for Proposals, the ad hoc Evaluation Committee selected AFLAC Columbus, GA as the most favorable proposal for Lake County for Section II, Accident and Cancer Supplemental Insurance and Healthsmart, Lisle, IL as the most favorable proposal for Section I, III and IV, Section 125 Flexible Spending Program Services, COBRA administration and Retiree billing administration.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that the Purchasing Agent is authorized to negotiate and enter into a contracts with AFLAC, Columbus, GA for employee-paid Accident and Cancer supplemental insurance plans and Healthsmart, Lisle, IL, in an estimated amount of $30,000 per year, plus renewals, for Section 125 Flexible Spending Program Services, COBRA and Retiree Billing Administration for plan year 2011 (January 1 through December 31), charging the costs thereof to account 730-1450010-74090.
