R E S O L U T I O N
WHEREAS, each year, the Division of Transportation undertakes patching of its concrete pavements to preserve surface quality, including repairing damaged manholes, catch basins, curbs and gutters; and
WHEREAS, this Lake County Board, by prior resolution at its meeting of March 8, 2016, appropriated $120,000 of Motor Fuel Tax funds for concrete pavement patching; and
WHEREAS, the 2016 concrete pavement patching is under contract with A Lamp Concrete Contractors, Inc., of Schaumburg, Illinois, in the amount of $97,931; and
WHEREAS, additional locations requiring concrete pavement patching were identified after the contract was awarded; and
WHEREAS, a supplemental appropriation of funds is necessary for the additional locations; and
WHEREAS, the Lake County Purchasing Ordinance requires the Public Works and Transportation Committee and the Finance and Administrative Committee to approve change orders with cumulative totals above 10 percent of the original contract.
NOW, THEREFORE BE IT RESOLVED that a supplemental appropriation of $20,000 of Motor Fuel Tax funds is hereby authorized for the 2016 concrete pavement patching, and designated as Section 16-00000-10-GM.
BE IT FURTHER RESOLVED that this resolution being presented to the Lake County Board for approval, the Public Works and Transportation Committee, and Finance and Administrative Committee hereby have approved Change Order Number Three, in the amount of $31,674.95.
BE IT FURTHER RESOLVED that this supplemental appropriation be processed by the County Engineer for final approval of the Illinois Department of Transportation in accordance with the “Agreement of Understanding” between the Illinois Department of Transportation and Lake County, dated January 18, 2013.
