Staff Summary
• The $162,500 Illinicare grant will be used to fund a new part-time position, computer, supplies, and mileage reimbursement.
• This position will be responsible for identifying, implementing and ensuring sustainability of workflows to improve the provision of care as related to value-based care operations and will be responsible for data collection, data validation, and accuracy of UDS and HEDIS reporting and identifying, implementing, and ensuring sustainability of workflows related to these quality metrics and their improvements.
• The $162,500 in additional grant funding has not been previously appropriated because the grant resources were secured after the County’s adoption of the Fiscal Year 2024 budget.
• If grant funding for the positions and services end, and new funding is not secured, the positions and services will be eliminated.

RESOLUTION
WHEREAS, the Lake County Health Department and Community Health Center has received a grant increase delivered by our Clinical Operations Unit; and
WHEREAS, these dollars will provide a legitimate increase in services to Lake County with no impact on local taxes; and
WHEREAS, this grant will be used for one new position that will be eliminated if or when this grant’s funding is no longer available and no other funding source outside of property tax dollars has been identified; and
WHEREAS, this grant amount totaling $162,500 has not been previously appropriated, having been awarded after adoption of the Fiscal Year 2024 budget; and
WHEREAS, the proposed appropriation will be fully offset by the above referenced items thereby not requiring the expenditure of local tax revenue.
NOW, THEREFORE, BE IT RESOLVED, by this County Board of Lake County, Illinois, that an emergency appropriation in the amount of $162,500 is hereby authorized in the Health Department budget as identified on the listing of expense and revenue budget modifications attached hereto and made part hereof; and
BE IT FURTHER RESOLVED, that the revenue budgets as identified on the attached listing be modified as provided by increased funding from the following:
Illinicare - Health Grant, $162,500.
