R E S O L U T I O N
WHEREAS, Arlington Heights Road, County Highway 79, is a designated route on the county highway system; and
WHEREAS, it would be beneficial for the safety and welfare of the motoring public if the bridge deck beams of the bridge carrying northbound Arlington Heights Road over Buffalo Creek could be replaced; and
WHEREAS, Phase II design engineering services are required for the proposed improvements; and
WHEREAS, this Lake County Board, at its meeting of January 12, 2010, authorized the execution of a Phase II design engineering agreement with MACTEC Engineering & Consulting Inc., Chicago, Illinois, for said Phase II engineering services and appropriated $50,000 of County Bridge Tax funds to provide for these engineering services; and
WHEREAS, this Lake County Board, at its meeting of April 12, 2011 approved the expansion of Phase II engineering services to provide for IDOT and Lake County DOT plan review comments and for the ongoing inspection and monitoring of deteriorating bridge deck conditions at a cost of $8,376; and
WHEREAS, it is necessary to further expand Phase II engineering services to provide for the consultant review of shop drawings for construction documents at a cost of $12,975; and
WHEREAS, Lake County and MACTEC Engineering and Consulting Inc., Chicago, Illinois, are desirous of entering into an Amendment #2 to the agreement for the supplemental Phase II design engineering, a draft copy of which is attached hereto.
NOW, THEREFORE BE IT RESOLVED, by this Lake County Board, that the County Board Chair, the County Clerk, and the County Engineer of Lake County are authorized and they are directed to execute a second supplemental agreement for consulting engineering services between Lake County and MACTEC Engineering & Consulting Inc., Chicago, Illinois, providing for said additional engineering services to provide for the review of shop drawings for construction documents at a cost of $12,975, a draft copy of which is attached hereto, and such authorization shall include any necessary subsequent amendments.
BE IT FURTHER RESOLVED the County Engineer shall transmit in writing the final agreement to be executed by the County Board Chair and the County Clerk.
BE IT FURTHER RESOLVED that this agreement be administered in accordance with Chapter 605, Act 5, Section 5-205.2 of the Illinois Compiled Statutes without further Board action providing the final contract cost chargeable under the funds appropriated does not exceed the appropriations.
BE IT FURTHER RESOLVED that this supplemental agreement, when executed in its final form, be submitted to the Illinois Department of Transportation for their approval and final processing.
BE IT FURTHER RESOLVED that a supplemental appropriation of $15,000 of County Bridge Tax funds for the review of shop drawings for construction documents is also authorized and designated as Section 05-00193-04-BR.
