Lake County Health Department and Community Health Center (LCHD/CHC) received grant funding from the Health Resources and Services Administration (HRSA) for the Uniform Data System Plus (UDS+) program for $46,032. The performance period is from June 5, 2024, to January 31, 2025. The purpose of the funds is to support high-quality UDS+ data submissions consistent with HRSA’s UDS Modernization Initiative. Funds will be used to purchase interoperability engine software.
