Title
Service Site Scope Change - North Chicago Health Center - Burke
Body
As a federally qualified health center (FQHC), the Lake County Health Department and Community Health Center (LCHD/CHC) is required to maintain a scope of project with the Health Resources and Services Administration (HRSA). The scope defines the approved service locations, services, providers, service areas and target populations served with section 330 grant funds. LCHD/CHC is responsible for updating this scope of project as organizational services change.
LCHD/CHC’s North Chicago Health Center (NCHC) had a 4-inch water line burst in the attic which subsequently flooded the NCHC in the late evening hours of January 22, 2024. The NCHC had to immediately close and its medical and dental providers, their care teams, behavioral health, and WIC services were relocated to the Belvidere Medical Building (BMB) to assure ongoing, comprehensive, and safe continuity of care for our clients.
LCHD/CHC Leadership is estimating that the NCHC will be closed for 9 months to complete the remediation and restoration along with safety and infection control mitigation.
This service site scope change of the closure of the NCHC during the remediation and restoration must be approved by the Governing Council. Once approved by the Governing Council, LCHD/CHC will submit a formal service site scope change request to HRSA.
Suggested Motion
To approve the service site scope change of the temporary closure of the North Chicago Health Center during the process of remediation and restoration as presented.
