Lake County Health Department and Community Health Center (LCHD/CHC) received an increase in grant funding through the Illinois Department of Human Services for the Supported Residential program in the amount of $7,316. The period of performance is from July 1, 2023, to June 30, 2024. The program provides support and services to individuals with a moderate level of mental illness or disability. The goal is to assist individuals in achieving self-sufficiency or transitioning to a less restrictive residential setting. This funding will offset existing staff salary and fringe.
