Title
Clinical Risk and Patient Safety Management - Koppit
Body
The Lake County Community Health Center Governing Council is required to annually review the risk management activities performed in response to the programmatic goals and objectives set within the Clinical Risk and Patient Safety Management Plan (CRPSM).
The CRPSM Plan was established to reduce system-related errors and potentially unsafe conditions by implementing continuous improvement strategies in support of an organizational culture of safety. The program addresses clinical, operational, and patient safety risks that support the expectation that all staff are:
• knowledgeable about and participate in risk management activities
• assist with the implementation of recommended improvements
• identify risk events and opportunities for improvement
The CRPSM Plan also delineates the support for the establishment of a culture within LCHD/CHC that emphasizes:
• implementing evidence-based best practices
• learning from error analysis
• providing constructive feedback and learning opportunities rather than blame and discipline
• understanding that incident reporting will improve the system and prevent future occurrences
The CRPSM Plan, Risk Training Plan, and report of activities for 2023 are attached for the Council’s review.
Suggested Motion
To approve the Clinical Risk and Patient Safety Management Plan as presented.
